Make a referral

If you’re thinking of making a referral for you and your child, it’s a great idea to come and visit us to see all of our wonderful facilities, and to find out a little more about the support we offer families.

We know that it may seem daunting to visit a hospice for the first time but our building is genuinely filled with love and laughter; every day is all about creating special memories, and making the most out of every moment.

You can come for a tour of the hospice, led by a member of our Care Team. you’ll be able to take a look at our fantastic accommodation as well as all the activities we have on offer. It’s also a great chance to ask any questions, and meet some of the other families who already use our services.

To arrange a visit, please contact our Family Support Team on 01909 517 360 and we'll be happy to talk you through the steps.

How to make a referral

Having a child with complex medical needs can sometimes feel overwhelming, so we like to keep our referral process as simple as possible.

We accept new referrals up to your child’s 19th birthday, as long as you live in our catchment area; South Yorkshire, North Nottinghamshire, North Derbyshire and North Lincolnshire. Referrals can come from healthcare professionals, or you can self-refer.

Once you have filled out a consent and referral form, we’ll be able to get your child’s medical records and we can begin to put together a care plan for your child. Our panel meets every four weeks to discuss new referrals.
